In accordance with Texas law House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, the following information is provided by Restore Clarity Counseling to help consumers understand their right to:

Request Your Health Care Records

You have the right to request a copy of your mental health records.

To request your records, please submit a written request to Restore Clarity Counseling using one of the following methods:

To request your records, please:

  1. Submit a Written Request - You may make your request in writing by email, mail, client portal, or in person during business hours.

  2. Provide Identification - Include your full name, date of birth, and contact information to verify your identity.

  3. Specify the Records Needed - Indicate which records you are requesting and the date range, if applicable.

  4. Allow Processing Time - We will respond to your request within 15 business days as required by Texas law.

Records will be provided in accordance with Texas law and applicable privacy regulations. If you have questions about accessing your records, please contact our office for assistance.

Under the Health Insurance Portability and Accountability Act (HIPAA) and Texas law, you have the right to inspect, obtain copies of, and request amendments to your protected health information. There may be reasonable fees associated with copying and mailing your records.

File a Consumer Complaint

Important Note: Your complaint is subject to the Texas Open Records Act meaning complaints and related information filed with the Consumer Protection Division are open to the public.
